Important Booking Information

  • I will confirm your appointment within 12 hours. 
  • After I confirm your appointment, you will receive two emails from me:
    • Your confirmed appointment details
    • A link to the intake/consent forms and instructions for how to prepare for the consult. If you do not receive this email, please check your Spam folder. The intake and consent form is required and ideally completed at least 24 hours before your appointment.
  • If your baby is having weight gain issues or if you have concerns regarding your milk supply, you would most benefit from an In-Home Consult
  • If your baby is less than 1 week old and/or having weight gain concerns, and you are unable to schedule within my online availability, please contact me directly. I will do my best to fit you in!

Note: Due to the nature of home visits and travel, I may need to shift your appointment time. I will email you directly if this is necessary. I must have a full address, including city/town, in order to confirm your appointment.

Payment & Insurance Information

  • I am in-network with Aetna/Meritain and United/UMR, and I am an out-of-network provider with all other insurance companies. 
  • If you have Aetna or United insurance, I will bill your insurance plan directly for both lactating parent and baby. It is your responsibility to verify that I am an in-network provider for your plan. If your baby does not have Aetna or United insurance, there will be a fee ($50 for Aetna, $100 for United) that is not eligible for in-network insurance coverage. I will supply you with a Superbill which you can submit for a reimbursement request to their insurance company, but I cannot guarantee reimbursement.
  • Payment for Self Pay appoitments is due at time of service. I will supply you with a Superbill which you can submit for a reimbursement request to your insurance company, but I cannot guarantee reimbursement. Most Massachusetts health insurance plans will reimburse for lactation care. Click here for more information on insurance coverage & reimbursement.
